| 内容記述 | 今回我々は比較的まれであり，観血的療法を選択することの多い中手指節関節（以下，MCP関節）脱臼を伴った示指基節骨骨折の症例に対して，非観血的療法を行い良好な結果を経験したため報告する．症例は22歳女性，柔道の受け身の際に受傷．右第2MCP関節の脱臼が疑われたため応急処置として徒手整復術を行ない，その後整形外科クリニックを受診した．診察時，右第2MCP関節部の疼痛と腫脹を認めた．X線上，脱臼を認めず，右示指基節骨骨折を認めた．骨折の徒手整復術を行ない，関節面に骨折線が及んでいないことから非観血的療法を選択した．同日に示指と中指の副木固定を行い，1週間後に自動屈伸運動を行う早期運動療法（以下，ナックルキャスト）を行った．ギプス固定期間は受傷後4週間であり，骨癒合も認め，回旋変形なく，関節可動域も良好であった．7週間後の握力（健側28kg，患側21kg）は健側比75％，受傷後12週間後の握力（健側26㎏，患側24㎏）は健側比94％となった．今回の非観血的療法においても良好な結果が認められた．本症例では徒手整復後に非観血的療法を行い良好な結果を得たが，徒手整復が可能であっても経過治療中に再脱臼，または骨折部の再転位を生じた報告もあり，注意深く経過を見ることが必要である． | |||||||||||||||||||||||||||||||||||

| 内容記述 | Dislocation of the index metacarpophalangeal joint with fracture of the proximal phalanx is relatively rare. We had a case of the dislocation of the metacarpophalangeal joint with fracture of the proximal of the index finger of a 22-year-old woman. X rays showed a fracture of the proximal phalanx of the index finger. We chose a noninvasive therapy. The woman with the fracture was treated using a knuckle cast after closed reduction. The knuckle cast was applied for four weeks. The patient’s progress was followed up for twelve weeks after the injury. X rays showed bone union. Rotational deformities did not occur, and a good range of motion was preserved. Grip strength decreased (94 % compared with the opposite side). The knuckle cast ensured stable fixation and enabled early range of motion to prevent hand stiffness． |
