今回の研究目的はMedial meniscus extrusion（MME）が良好群の場合，MMEとKnee injury and Osteoarthritis outcome score（KOOS）および膝伸展筋力がどのような関係性を示しているかを調査することである．対象者はMMEの測定値が3mm以下で既往歴無しの条件を満たした18名（男性3名・女性15名）とした．MMEは超音波画像診断装置を用い内側側副靱帯（MCL）の長軸上で内側半月板を描出して計測した．被験者の肢位は片脚立位で膝関節は伸展位とした．MME測定肢の膝伸展筋力の計測はハンドヘルドダイナモメーター（HHD）を使用して実施した．また膝関節の主観的評価としてKnee injury and osteoarthritis outcome score（KOOS）を実施した．統計処理はMMEに対する膝伸展筋力とKOOSとの相関をスピアマンの順位相関係数を用いて実施した．
結果はKOOSの痛みの項目のみMMEと弱い相関関係が確認されたが，それ以外の項目とは相関が確認されなかった．これにより，MMEが3mm以内であれば軽度の疼痛が存在しても膝伸展筋力やKOOSは相関せず，比較的良好な状態を保てていることが示唆された． |

The aim of this study was to investigate the relationship between medial meniscus extrusion (MME), the knee injury and osteoarthritis outcome score (KOOS) and knee extensor muscle strength in patients with good MME. The study included 18 participants (three males and 15 females) with MME measurements ≤3 mm and no history of knee injury. MME was measured using an ultrasound imaging device by visualizing the medial meniscus on the long axis of the medial collateral ligament (MCL). Participants were placed in a one-leg standing position with the knee joint in extension to measure the muscle strength of the MME-measured limb. Knee extensor muscle strength was measured using a hand-held dynamometer. The KOOS was used to subjectively evaluate the knee joint. Statistical analysis was performed using Spearman's rank correlation coefficient to correlate knee extension muscle strength to MME and KOOS.
Only the pain item of the KOOS showed a weak correlation with MME. This suggests that knee extensor muscle strength does not correlate with KOOS in patients with MME ≤3 mm. Although mild pain exists, patients could maintain a relatively good position. |
