 note = {今回われわれは挫創を伴う転位の著明な手指基節骨基部骨折に対し，中手指節関節（以下，MP関節）屈曲位ギプス固定を行い，その直後から固定下で積極的に手指遠位指節間関節（以下，DIP関節），近位指節間関節（以下，PIP関節）の自動屈伸運動を行う早期運動療法（ナックルキャスト）を行い良好な結果を得た症例を経験したため報告する．症例は64歳，女性．平成25年5月1日転倒し左手を地面につき受傷，その後左手痛が持続するため，同日に上青木整形外科を受診した．X線画像上左小指基節骨基部骨折を認め，正面20°，斜位39°の角状変形を認めた．また左小指MP関節部掌側に挫創を認めた．転位が著明で挫創を合併する骨折の場合，単純骨折よりも軟部組織損傷が高度であるため早期運動により炎症が沈静化せず，感染を併発する可能性があり，時に治療に難渋する場合もある．しかし本症例では，短期間での固定でも不可逆的な拘縮をきたす可能性があるため，今回は早期運動療法を選択した．骨折部整復の後に，小指と環指とのbuddy taping後，創処置を行いナックルキャストを施行した．キャスト施行直後から積極的な左小指の屈伸運動を指導した．創処置を継続するため左小指MP関節部掌側のキャスト部分はカットした．ギプス固定期間は4週間であり，骨癒合を認め，回旋変形もなく可動域も良好であった．挫創も2週間で治癒し特に感染兆候も見られなかった．, Hand fractures are common injuries. Early mobilization exercise in an intrinsic plus position (the knuckle cast) is said to be good treatment for fractures of the proximal phalanx and metacarpal bones. 
We reported on the early mobilization exercise of fracture of the base of the proximal phalanx with severe soft tissue injury and severe angular deformity using the knuckle cast of a 64-year-old woman.　X rays showed fracture of the base of the proximal phalanx and severe angular deformity. The woman with fracture of the base of the proximal phalanx with contused wound and severe angular deformity was treated with the knuckle cast after closed bone reduction. Bone union was seen. No signs of infection in bone and soft tissue were seen. Rotational deformities did not occur．As a result, a good range of motion was preserved. The knuckle cast was suitable enough to permit early range of motion and functional rehabilitation.
We consider that the knuckle cast had been a reliable form of early mobilization exercise for treating fractures of the proximal phalanx with severe soft tissue injury.},
